• The final qualification sheet
• Caste certificate
• Income certificate
• Bank passbook
• Payment receipt number
• Annual non-refundable amount
• Registration number
• Aadhaar card number
• Recent passport size scan photo
• Candidates for renewal – Kindly use previous year registration number to enter renewal section and enter fresh details

• note- Those who have registered themselves in previous year should renew the current form considering them as old user and renewal candidate.

Offline submission for UP Scholarship form 2024-25

• After filling the online scholarship form, candidates should take the printout of the complete form and after that attach the relevant documents (mentioned above) and send it to the college/school/institute before the due date.
